Job Description - California Licensed Care Manager
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.
Key Responsibilities
Evaluate member needs and recommend plans for optimal care outcomes
Develop and collaborate on ongoing care plans with providers and community resources
Monitor member progress and update care plans as necessary to meet goals
Required Qualifications
Degree from an Accredited School of Nursing or Bachelor?s degree in Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State Licensure required
Must reside in California for remote position
